Back-to-School Forms

For the 2020-2021 school year, we will be shifting required back-to-school forms and student medical information to Magnus Health SMR (Student Medical Record). Note: Magnus also facilitates our daily health screenings for any individuals entering the building. 

You should receive an email from Magnus by end of day on August 10, 2020, providing your username/password to access your child(ren)’s Magnus Health account(s). If you did not, please email service@magnushealthportal.com.

You must enter the information required within your Magnus Health SMR account for each child attending Commonwealth. 

Update on Physical Exams

Please note that the state of Massachusetts recently updated its guidelines about physical exams for students: you can now send in your child’s most recent physical within the last four years. 

Forms on Magnus include:

  • Annual Physical Exam Form
  • Audiovisual Recording Notice and Consent
  • Chaperone Policy
  • Driving Consent Form
  • Handbook Acknowledgement Form (Parent-Student Handbook here)
  • Immunization Record
  • MBTA Student Pass Enrollment
  • Over-The-Counter Medication
  • Permission to Treat
  • Prescription Medication Form
  • Universal Authorization Form
  • Vital Health Record

To complete these forms, please log into Magnus and follow the steps provided. 

 


 

Informed Consent

Required for All Students Who Will Attend In-Person Classes

If you intend to send your child(ren) to our building for in-person classes, you must complete an Informed Consent Form indicating you understand the school’s approach to on-campus learning during the COVID-19 pandemic and the related risks of exposure to the virus, and consent to your child attending school in person.

No student will be allowed to enter the building without having a completed informed consent form on file.

Consent to Collect Coronavirus Information 

Required for All Students Who Will Attend In-Person Classes

All in-person students must participate in weekly COVID testing, provided by the school. (Please see our Fall 2020 page for details.) If you intend to send your child(ren) to our building for in-person classes, you must complete a Consent to Collect Coronavirus Information indicating you understand the information we are collecting and sharing pertaining to these COVID-19 tests.

No student will be allowed to enter the building without testing and having a completed form on file.

  • Grandparent Information Form: Extended family are part of the Commonwealth community too! Fill out our Grandparent Information Form to ensure they receive invitations to school events and news throughout the year.
