Each year, the back-to-school forms listed below are required for each student to matriculate and begin school in September. Some forms are required of all students and others are required for some students.
Please read the following instructions carefully and submit the forms electronically as indicated or by mail as requested. If you have any questions regarding the forms, please contact Stephanie Poynter at firstname.lastname@example.org.
All forms are due by August 15, 2018.
For all students: Please complete all five required forms listed below by August 15, 2018.
- Handbook Acknowledgement Form
Please read the Parent-Student Handbook before completing the Handbook Acknowledgement form.
- Physician's Medical Report
Please provide a copy of the form provided by your physician's office. Please make sure it includes all required immunizations. Note that Commonwealth requires each student to undergo a physical exam each year and a current physician's report must be submitted to the front office before the start of the school year. - Massachusetts Pre-participation Head Injury/Concussion Reporting Form (pdf) Please download and send to firstname.lastname@example.org, or mail to Stephanie Poynter at the school address.
For some students: Please complete the following form(s) as needed by August 15, 2018.
In addition to the physician's medical report, the student's physician must fill out and return a Food Allergy and Anaphylaxis Emergency Care Plan, signed by the physician and parents. This form is required of all students with known allergies. Please download and include this form with all medical forms to email@example.com, or to Stephanie Poynter at the school address.
All Commonwealth School communications will be shared with the contact information the families have provided upon enrollment. This form is required if your student and/or family has had a change of address in the past year,
Some of our students opt to commute by car to school and other school related events. This form is required of all student drivers.
Please complete the following additional forms as needed by August 15, 2018.
